Hospital and Nursing Home Patient Advocacy:
Our firm often acts as a liaison between family members, patients/residents,
and the staff at both hospitals and long term care facilities.
We participate in "care team" meetings, both when
residents first enter facilities, as well as when there is
a breakdown in communication during a resident's stay. In
addition, we assist the family, patient/resident, and social
workers with discharge planning and transfers.
